Use this form ONLY for PA-related questions/issues.

Use this form when the PA queue is very high, and you are not able to get an agent on the line, or when the PA department is closed due to Business hours. Advise the patient of the TAT within 72 business hours

If the caller is not the patient and not a payor (ins company), you must obtain verbal authorization from the patient and document it in LIMS and AMD. No exceptions.
