Account Modifications & Data Entry Error

Before any modification can be done, the account must be HIPAA verified.
If the patient provides a different mailing address, phone number or email address than what we have on file you should send a Syncro CRM “Demograhics Update” to have the information updated.
For misspelled first and last name or DOB, you must look in LIMS and the requisition form to verify if it could have been a data entry error.

If the information provided by the patient matches the requisition form, there’s no need to request a copy of the patient’s driver’s license; you can proceed with  HIPAA verification and submit Syncro CRM “Demographics Update”

IF THE FIRST, LAST NAME OR DATE OF BIRTH DOESN’T MATCH WITH THE REQUISTION FORM, advise the patient to send a copy of his / her driver’s license along with the case# on the subject line to [email protected]

If the patient can send it with you on the line, please ask which e-mail address they used to send it and ask your CSS to search it so changes can be made immediately, but it will take 2 business days to take effect. 

IF THE INFORMATION DOES NOT MATCH, YOU CANNOT CONTINUE WITH THE AUTHENTICATION.
